This looks fun. I can't imagine that it works particularly well.
The dual register key holes are interesting. Maybe one hole is used for Bb and the other for A somehow.

A few of these or something similar have shown up for auction a few times over the years. The ones I'm thinking of had some sort of metal sleeve or something inside the bore and had cracked as the wood shrunk, unfortunately.
